Instructions for filling out forms:
IMPORTANT: You must print out forms #1 – 4 below and bring them with you to your first team meeting. If you have any questions, please contact the Athletics Department at (315) 684-6072.

5. General Amateurism and Eligibility Form for International and Select Student-Athletes – Division III **

* If you are under 18 years old you will need a parent or guardian's signature on this form.

** If you are an international student looking to participate in intercollegiate athletics, please fill out the General Amateurism and Eligibility Form for International and select Student-Athletes. As an intercollegiate athletics participant it is vital that you check your health insurance status to be certain your family policy provides for international coverage and/or that you have established medical insurance through the purchase of the Morrisville State College health insurance plan to cover your athletic participation.

If you are looking to transfer to another institution, you will need to fill out the Permission to Contact: Self Release Form and submit that form to the institution you are looking to transfer to.
